Job Overview

An exciting opportunity has arisen to join our multidisciplinary community team. You will coordinate and delegate patient care across boundaries within secondary and social care settings. This role involves managing a caseload of patients with complex health needs, aiming to prevent deterioration, hospital admissions, and prolonged hospital stays.

We currently have openings at Brighton General Hospital for the Brighton & Hove East and South community nursing teams.

Main Duties

• Accountable for a patient caseload, delegating responsibilities within the multidisciplinary team.
• Conduct advanced assessments of physical, mental, and social care needs to provide clinical care and health promotion interventions to avoid unnecessary hospital admissions.
• Interpret clinical signs, symptoms, and diagnostic tests to develop treatment plans in collaboration with medical practitioners. Provide advanced clinical advice to improve patient outcomes.
• Support patient independence through self-management strategies.
• Develop and analyze audits, interpret data, and implement findings to enhance clinical practice.
