Job description

Aldershot Community nursing team are seeking an enthusiastic and experienced B6 nurse or District Nurse SPQ with a warm personality and great people skills.

You must have leadership experience and be able to demonstrate innovative practice and be informed of current community nursing practices. You must also have post-qualifying experience with either community nursing skills and competencies or transferable skills and competencies.

You need effective management skills to take responsibility for caseload management of housebound patients with multiple nursing needs in the community, with support from the Team Leader.

You will be responsible for ensuring high-quality, evidence-based care for patients, their families, and carers in the community. This includes acting as a role model for staff, sharing learning, and developing practice.

Must be a driver with access to a vehicle for work; mileage is claimable via the trust expenses system.

Key Responsibilities:
  1. Manage the day-to-day running of a community nursing team in conjunction with the Team Leader.
  2. Ensure the community nursing team functions at a high level, providing excellent patient care.
  3. Be part of a supportive team of other B6 nurses covering the north east of Hampshire, providing a 7-day service from 8am to 8am.

Key Duties:
  1. Manage the community nursing team daily operations.
  2. Undertake holistic assessments for new patients.
  3. Complete assessments such as Purpose T, MUST/MUAC, wound assessments, ensuring staff compliance.
  4. Manage chronic wounds (e.g., leg ulcers, pressure ulcers) using best practices and NICE guidelines.
  5. Manage short-term wounds (e.g., surgical wounds).
  6. Handle patients with JVAC drains, lantern drains, pig tail drains, and flushes.
  7. Care for PICC lines and chemotherapy disconnections (training provided).
  8. Care for other central lines like Mid lines and portacaths (training provided).
  9. Administer intravenous antibiotics in the community.
  10. Ensure pressure-relieving equipment is in place or referred for patients.
  11. Communicate effectively with GPs and build relationships.
  12. Participate in weekly MDT meetings in the absence of a community matron.
  13. Advocate for patients as needed.
