Job summary

Avon Search is seeking an experienced Reablement Officer in Bournemouth to lead a team delivering short-term rehabilitation and reablement in a community setting. This full-time role is 40 hours per week with early/late shifts across 7 days, and a salary of £37,000 per annum.

You will oversee a team of Reablement Assistants, coordinate workloads, provide supervision, and work with healthcare professionals to achieve client goals while maintaining high standards of care and documentation.

Qualifications

• NVQ Level 3 in Health & Social Care (or equivalent) – Essential.
• Experience in health, social care, rehabilitation or reablement services.
• Experience supervising, leading or mentoring care staff.
• Full UK driving licence with access to a vehicle.
• Business use motor insurance is essential.

Responsibilities

• Managing and supporting a designated team of Reablement Assistants.
• Allocating workloads and planning staff schedules.
• Providing supervision, support and guidance to care staff.
• Supporting recruitment, appraisals and identifying training needs.
• Ensuring staff deliver high-quality, outcome-based reablement support.
• Completing first visits and review visits with clients.
• Undertaking risk assessments and providing advice to clients, families and professionals.
• Supporting individuals to achieve agreed goals and maintain independence.
• Liaising with healthcare professionals and other services involved in client care.
• Ensuring compliance with policies, procedures, CQC Fundamental Standards and the Care Act 2014.
• Maintaining accurate electronic records and case documentation.
• Supporting safe working practices and ensuring appropriate equipment and PPE is available.
