NHS Professionals is seeking a Community Psychiatric Nurse for community-based work across London NHS Trusts. The role involves assessing patients referred with mental health difficulties by phone or in person and agreeing the least restrictive care pathway that meets each patient’s needs.

The post requires recent paid community psychiatric nursing experience in the UK and an in-date NMC registration. Shifts are flexible, with collaboration across nursing, OT, medical, psychology and admin teams.

Formación

• NMC registration must be in date.
• Minimum 6 months paid community psychiatric nurse experience in the UK within the last 3 years.
• Full UK driving licence and access to own vehicle.

Responsabilidades

• Assess patients referred with mental health difficulties by telephone or face to face
• Identify appropriate care pathways and select the least restrictive option
• Signpost patients to primary and tertiary care providers
• Raise concerns with senior staff for support and management
• Complete documentation and plan patients’ care
• Present cases to the multidisciplinary team to discuss care pathways
• Liaise with nursing, occupational therapy, medical, psychology and admin colleagues

A bank post for a community psychiatric nurse, offered through NHS Professionals for the NHS Trusts it partners with.
