Hours: Full-time (37.5 hours per week, Monday to Friday)
Job Description: We are currently recruiting for an experienced
Band 6 Community Psychiatric Nurse (CPN) to join a dedicated Adult Community Mental Health Team (CMHT) based in
Aylesbury, working through
Oxford Health NHS Foundation Trust.
The successful candidate will play a vital role in providing high-quality mental health care to service users in the community, acting as a
care coordinator under the Care Programme Approach (CPA).
Key Responsibilities:- Manage a defined caseload of clients with complex mental health needs, providing comprehensive assessments, risk management, and care planning.
- Act as care coordinator, ensuring effective delivery of evidence-based interventions in line with Trust policies and recovery-focused care.
- Liaise and collaborate with a range of internal and external professionals, including social care, GPs, housing, and voluntary sector providers.
- Participate in regular multi-disciplinary team meetings, CPA reviews, and safeguarding procedures.
- Administer and monitor prescribed medications where necessary, including depot injections.
- Maintain accurate clinical documentation using the Trust’s electronic records system.
Requirements:- Current NMC registration as a Registered Mental Health Nurse (RMN)
- Minimum of 2 years’ post-registration experience, ideally within a community mental health setting
- Experience in care coordination and working within the CPA framework
- Strong communication, risk assessment, and clinical decision-making skills
- Ability to work autonomously as well as part of a multidisciplinary team
- Full UK driving licence and access to a vehicle (preferred but not always essential)
