We have an exciting opportunity for a Community Prevention Officer to join us!

This is a full-time, permanent role with hybrid working and a salary of £37,526 - £41,613 per annum.

This is an excellent opportunity for an enthusiastic Community Prevention Officer to join our Systems, Performance and Communitiesteam.

The Role:

We are looking for a passionate and community-focused Community Prevention Officer to join ourSystems, Performance and Communities team. Reporting to the Community Prevention Manager, you will play a key role in connecting residents with local resources to enhance their independence and wellbeing.

Your role will involve:

  • Acting as a central point of contact for referrals from health and social care.
  • Developing personalised community support plans for residents.
  • Collaborating with local groups and services to match residents with appropriate support.
  • Supporting community capacity building and integration across sectors.
  • Monitoring outcomes and contributing to service improvement.

What we are looking for:

  • Experience working directly with communities and in partnership with stakeholders.
  • Knowledge of local community resources and the voluntary sector.
  • Strong communication and interpersonal skills.
  • Excellent organisational and time management abilities.
  • Confidence using IT systems and managing referrals.
