Community Practitioner

The Personality Disorder Hub Team, are recruiting for an experienced Community Practitioner or those wishing to develop into the role. This is an opportunity for individuals who are interested in developing specialist skills in managing complex risk and promoting safety. Successful candidates will be provided with training, supervision and guidance to help develop their specialist skills.

The Team itself is central to a well-developed and cohesive series of wider pathway developments for service users with personality difficulties.

Our community practitioners work into different localities across the region. We currently have two vacancies, one in the South of the region, covering Sunderland and South Tyneside and the other within the North of the region, covering Northumberland and North Tyneside.

Main duties of the job
  • Specialist care co-ordination and treatment for a small number of service users with complex personality difficulties, co-existing conditions and high risk presentation.
  • Structured Clinical Management (SCM), adapted trauma stabilisation and to a lesser extent Mentalisation Based Treatment (MBT) during the most unstable phase of a service users care pathway.
  • Focus will be on providing safety, stabilisation, containment and emotional regulation and control.
  • Scaffolding to a wide range of community teams, inpatient staff and third sector providers.
  • The Personality Disorder Hub Team is part of a series of wider pathway developments for service users with Personality Disorder aimed at improving overall care, in partnership with a wide range of stakeholders. On this basis, Community Practitioners can expect to interface with many services both within and outside of CNTW.

Person Specification
Education and Qualification
  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.
Knowledge and Experience
  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
  • 12 to 24 month band 5 qualified nurse experience dependent upon service need.
Skills and Competencies
  • Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
  • Risk Assessment and Risk Management / Safety Planning skills.
  • Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance).
  • Demonstrate effective written and verbal communication skills.
  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
