Leeds Community Healthcare NHS Trust is seeking a Band 5 Physiotherapist to join a Short-Term Care Beds service on a permanent basis in Leeds. The role focuses on short-term rehabilitation to help patients regain independence after illness or injury and supports safe discharge planning.

You will assess, treat and rehabilitate adults with a range of physical needs, working in a supportive multidisciplinary team with ongoing supervision and development opportunities to grow clinical skills and

Responsabilidades

• Undertake physiotherapy assessments and develop patient-centred treatment plans.
• Deliver rehabilitation programmes to improve mobility, strength, balance and independence.
• Monitor progress and adapt plans to maximise recovery and safe discharge planning.
• Collaborate with Occupational Therapists, Nurses and Rehabilitation Assistants to provide holistic care.
