Qualifications

• HCPC registration as Physiotherapist (UK).
• Full UK driving licence required for community visits.
• HCPC registration is mandatory.

Responsibilities

• Deliver high-quality community-based physiotherapy interventions in patients' homes.
• Collaborate with an integrated health and social care service alongside OT's and support staff.
• Apply rehabilitation and orthopaedic experience to support recovery and independence.
• Assess, recommend and provide minor equipment to aid daily living and mobility.
• Conduct holistic assessments to address wider health needs.
• Complete falls risk assessments and implement prevention plans.
• Develop and review personalised treatment plans to achieve positive outcomes.
• Maintain accurate clinical records and documentation.
