Our team of Physiotherapists, Occupational Therapists, Therapy Assistant Practitioners, and Therapy Assistants offer assessment and rehabilitation to patients predominantly within their own homes. We work with patients to identify rehabilitation goals to improve independence and safety with mobility, function, and everyday activities. You will be welcomed into our friendly and established therapy teams and benefit from access to expert clinicians, receive regular clinical supervision, and be able to attend and participate in an in-service training programme with protected CPD time and a commitment to career progression and development.

Responsibilities:
  1. Manage a caseload of Community Therapy patients requiring assessment and rehabilitation.
  2. Triaging and prioritising your work area, balancing patient-related and professional demands.
  3. Provide leadership and training for junior staff and Therapy Assistants within the team and supervise physiotherapy students on placement.
  4. Work as an autonomous practitioner within the multidisciplinary team.
  5. Refer patients to health, social care, and voluntary agencies as appropriate following discharge from the team.
