Job summary

Waythrough is seeking an Outreach Worker for London to deliver community outreach services and support adults facing barriers to access. The role involves working across GP hubs, supported accommodation, and clients’ homes, applying person-centred care, needs assessments, and risk management plans to promote independent, sustained recovery.

You will collaborate with local agencies, attend multi-agency meetings, and help with signposting, education, training, and healthier lifestyles.

Qualifications

• Ability to deliver person-centred support to individuals with complex needs.
• Experience or understanding of outreach work within community settings.
• Strong communication and relationship-building skills.
• Ability to work collaboratively with multi-agency partners.
• Knowledge of safeguarding procedures for vulnerable adults and children.
• Good organisational skills and ability to manage case records effectively.
• Commitment to continuous professional development and learning.

Responsibilities

• Deliver outreach services in a range of community settings, including GP hubs, supported accommodation, and service users’ homes.
• Support the Project Manager and team in providing high-quality, person-centred care.
• Carry out needs assessments and develop support and risk management plans.
• Help service users live independently and work towards sustained recovery.
• Support tenancy sustainment and stability.
• Work with local agencies to address anti-social behaviour and improve engagement with services.
• Attend multi-agency meetings and collaborate with stakeholders.
• Promote the service, receive referrals, and provide signposting advice.
• Encourage service users to engage in education, training, and healthier lifestyles.
• Maintain accurate case records and update management systems.]
