Agincare Group is seeking a dedicated Outreach Support Worker to join our team in Bexley. In this role, you will provide high-quality support to individuals with learning disabilities and mental health needs, helping them achieve personal goals and live independently.

As part of a committed team, you'll support clients in their daily living activities, promote their engagement in the community, and maintain meaningful connections.

Qualifications

• Minimum of 12 months on your right to work.
• Full UK Drivers Licence is required.

Responsibilities

• Provide high-quality, person-centred support.
• Encourage clients to play active roles in their community.
• Support activities including personal care and budgeting.
• Communicate effectively using clients' chosen methods.
• Record and report changes in clients' health.
