Job summary

East of England Community Health and Care NHS Trust in Norwich is seeking an experienced Occupational Therapist to join our Community Therapy team at Norwich Community Hospital. You will work across Norwich Place with a varied caseload and a positive, person-centred approach.

The role requires flexible, independent working, good clinical reasoning, and strong communication skills as we support patients to maintain independence and quality of life in the community.

Qualifications

• Diploma or 1st level degree in Occupational Therapy, or equivalent experience.
• Current HCPC registration.
• Clinical educators course required.

Responsibilities

• Assess and prescribe care packages for patients with long-term conditions and rehabilitation needs.
• Work with integrated teams to facilitate early discharge and prevent unnecessary admission.
• Communicate effectively with patients and carers to maximise rehabilitation and understanding of treatment.
• Liaise with GPs, Social Services, inpatient teams and other professionals to coordinate care.
• Plan and organise specialist services within the integrated team.
• Ensure patient records are completed and input in SystmOne daily.
