Job summary

We Do Social Work is recruiting an Occupational Therapy Practitioner to join the West Locality Occupational Therapy Team on a one-year maternity cover contract. The role focuses on complex home assessments, enabling adaptations and providing specialist equipment to adults living in the community.

Qualifications

  • Must have HCPC registration as an Occupational Therapist.
  • Experience assessing adults in their own homes.
  • Experience recommending and providing equipment and adaptations.
  • Experience assessing complex equipment, including manual handling equipment.
  • Knowledge and experience of Disabled Facilities Grant (DFG) processes.
  • Experience working with adults with complex needs.
  • Strong assessment, report-writing and professional decision-making skills.
  • Excellent communication and multi-agency working skills.

Responsibilities

  • Complete comprehensive occupational therapy assessments in service users' homes.
  • Assess the functional needs of adults living in the community.
  • Identify risks and barriers to independence, safety and participation.
  • Recommend appropriate equipment, minor adaptations and major adaptations.
  • Assess and recommend complex equipment, including manual handling equipment.
  • Support applications and recommendations through the Disabled Facilities Grant (DFG) pathway.
  • Work with service users, families and carers to identify practical solutions that promote independence.
  • Provide advice and guidance regarding moving, handling and safe transfers.
  • Consider the most appropriate intervention based on the individual's needs, home environment and desired outcomes.
  • Liaise with housing, contractors, health professionals, social care colleagues and other agencies to coordinate adaptations and equipment.
  • Complete clear, accurate and timely assessments, recommendations and professional documentation.
  • Monitor and review equipment and adaptations to ensure they remain appropriate and effective.
  • Promote person-centred practice and maximise service users' choice, control and independence.
  • Manage a caseload of complex cases effectively and prioritise work according to risk and need.
