Wight Healthcare is seeking a Band 5 Occupational Therapist to join our Integrated Community Team in Pennington, England. You will assess and support adults in their homes and communities, delivering rehabilitation and enabling independence through a holistic, person-centred approach.

We are currently recruiting for an enthusiastic & motivated Band 5 Occupational Therapist to join our Integrated Community Team. You'll be joining a friendly and supportive team known for its collaborative approach and commitment to delivering high-quality, person-centred rehabilitation and enabling individuals to achieve their goals within their own homes and communities.
As a Community Occupational Therapist, you will work as part of a multidisciplinary team to assess, treat, and support adults with a wide range of health needs. You will provide holistic assessment and intervention, including rehabilitation, falls prevention, moving and handling assessments, postural management, equipment provision, and environmental adaptations to promote independence, safety, and wellbeing.
You will manage a varied caseload and work closely with colleagues across health and social care to support people to remain at home, prevent avoidable deterioration, and facilitate recovery following illness or hospital admission.
