Job summary

Cornwall Partnership NHS Foundation Trust is excited to offer an opportunity for a Registered Nursing Associate to join the Arbennek Community Nursing Team. The post is based at the Clays base but requires care delivery across the PCN.

You should demonstrate commitment to teamwork and delivering high standards of care, with the ability to travel independently across the area to fulfil requirements and to engage in ongoing professional development.

Qualifications

• Nursing Associate Foundation Degree.
• NMC registration.
• Previous experience of working within a health/care setting.
• Previous experience working within a Community Nursing Team.

Responsibilities

• Work with the Registered Community Nurses and other members of the multi‑professional team, providing and monitoring care.
• Contribute to ongoing assessment in line with service pathways.
• Monitor the condition and health needs of people within their care in partnership with the multi-professional team.
