Job summary

Somerset NHS Foundation Trust is seeking a compassionate Community Staff Nurse to join the District Nursing North Sedgemoor team. You will visit patients at home across Highbridge, Burnham and surrounding areas, delivering complex care with a patient-centred approach.

You will work flexible patterns with weekend cover where required, driving to patient homes. We welcome newly qualified nurses, offering preceptorship and continuing professional development in a supportive team.

Qualifications

• Relevant degree in Nursing or equivalent qualification.
• Professionally registered with the Nursing & Midwifery Council (NMC).
• Evidence of strong literacy and English language skills; continuing professional development.

Responsibilities

• Provide care to patients in their homes within the District Nursing North Sedgemoor team coverage area.

• Develop and implement holistic care plans and autonomous clinical decisions with support from Team Leaders.
• Support the Hospital @ Home initiative to prevent unnecessary admissions.

Are you a compassionate Registered Nurse (RN) seeking a new opportunity? We have Community Staff Nurse position available within the District Nursing North Sedgemoor team in Sedgemoor Neighbourhood covering Highbridge, Burnham, Cheddar, Axbridge and all the villages in between. Should you be successful the team cross-cover the whole area. The shift patterns are 8am-4pm, 9am-5pm 10am-6pm or 2pm-10pm. You will also work one in four weekends which include additional unsocial hours enhanced payments. We offer flexible working solutions where possible to ensure a work-life balance and part-time hours will be considered. You'll need to be a car driver with your own vehicle to visit patients in their own homes. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation. Main duties, tasks & skills required

As a community nurse, you'll provide care to patients in their homes, managing a range of complex health issues and collaborating with various other disciplines. You'll also work closely with the Hospital @ Home team, focused on preventing acute admission. You'll develop a broad range of nursing skills, including IV medication administration, palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management. You'll need to make autonomous clinical decisions and create holistic care plans, with the support of your colleagues and Team Leaders. Comprehensive training and guidance will be provided by our dedicated Clinical Lead Nurses.

Community Nurses work as part of a team providing high quality and compassionate care to individuals. The Community Nurse is an autonomous practitioner who is responsible for the holistic assessment of both complex and simple care needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team. They will support the multidisciplinary team in the delivery of high-quality care to enable individuals to achieve their person‑centred goals. The Community Nurse will take responsibility of the patients care pathway and non‑clinical duties in the absence of the Team Leader. They will have the understanding to address complex problems in their day‑to‑day work, reporting concerns to a Team Leader. The Community Nurse will be able to work effectively as part of a team and will always act within the limits of their competence, knowing who to ask for help and support as required. Community Nurses will operate within agreed ways of working, following the relevant standards, policies and protocols used in their workplace including the standards in the NMC Code, Professional standards of practice and behaviour for nurses and midwives.

Relevant degree in Nursing or equivalent qualification Appropriately registered with the Professional body as per post requirements Evidence of good standard of literacy and English language skills, Evidence of continuing professional development and to have undertaken post registration training, coaching and / or mentorship training, Knowledge and experience of current national policies affecting the NHS Knowledge of audit and research skills Understanding of clinical governance Knowledge of physical & Mental health needs or know where to access appropriate advice, Substantial experience in the required setting Ability to undertake the higher‑level clinical skills required for the specialist area Demonstration experience in the required setting Demonstrate clinical and managerial leadership ability Experience of working in multi‑agency services Experience of specialist working in District Nursing, Able to demonstrate clinical reasoning skills to assimilate information to make a clinical judgement regarding diagnosis and intervention Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, e‑mail, or written communication by dealing with incident reports or writing statements Compassionate - Open minded, treats colleagues, patients, carers and relatives with dignity and respect. Dexterity and accuracy required for, e.g., intravenous injections, syringe pumps and infusion, insertion of catheters and removal of sutures Recognising in yourself and others the impact that frequent exposure to distressing circumstances has on care and compassion and being able to respond professionally Ability to work autonomously Professional and patient focussed approach, acting as a role model to colleagues and junior staff members. Ability to manage own time and time of others Ability to take instructions and work as part of a team, motivate self and others Must be a car driver with a valid driving licence or have access to transport with appropriate business insurance to travel throughout the Trust as appropriate for role.
