Job summary

HCRG Care Group is seeking a Community Nurse (Band 5) to deliver high-quality, person-centred nursing in patients' homes from Amesbury Health Centre. You will work in line with national standards and collaborate with GPs and MDT members to assess, plan and deliver care, promoting independence and positive health outcomes.

You will require an NMC registration, be able to work autonomously within a multidisciplinary team, and have a full UK driving licence with car access.

Qualifications

  • NMC/1st level registered nurse.
  • Ability to work autonomously and within a multidisciplinary team.
  • Excellent communication and organisational skills; flexibility in approach.

Responsibilities

  • Assess, plan, deliver and evaluate nursing care in partnership with patients and carers.
  • Monitor progress and escalate changes in condition as needed.
  • Prioritise and manage a caseload according to patient needs.
  • Maintain accurate records using electronic and paper-based systems.
  • Deliver evidence-based care and contribute to quality improvement and audits.
  • Promote health, wellbeing and independence for patients and carers.
  • Participate in team meetings, handovers, supervision and mandatory training.
  • Support learning and development of students and colleagues.
  • Maintain safe environment through risk assessment and infection prevention controls.
  • Ensure safe use and maintenance of clinical equipment and stock.

Job Introduction

As a Community Nurse Band 5, you will deliver high-quality, person centred nursing care to patients in their own homes, working in line with national and local policies, professional standards, and the NMC Code. You will work closely with patients, families, GPs, and multidisciplinary colleagues to assess needs, plan and deliver care, promote independence, and support positive health outcomes. You will contribute to service development, health promotion, continuous improvement initiatives, and the education of students and colleagues, ensuring the highest standards of care are maintained across the service.

As a Community Nurse you will require excellent communication, organisation, and clinical skills, alongside a commitment to delivering compassionate care and continuously developing professional practice:

  • Assess, plan, deliver, and evaluate nursing care in partnership with patients, carers, and the multidisciplinary team.
  • Monitor patient progress, recognising and escalating changes in condition appropriately.
  • Prioritise and manage a caseload in line with patient need.
  • Maintain accurate and timely patient records using electronic and paper-based systems.
  • Deliver evidence-based care and contribute to quality improvement initiatives and clinical audits.
  • Promote health, wellbeing, and independence for patients and carers.
  • Participate in team meetings, handovers, clinical supervision, appraisals, and mandatory training.
  • Support the learning and development of students and colleagues.
  • Contribute to maintaining a safe environment through risk assessment, incident reporting, and adherence to infection prevention and control standards.
  • Ensure the safe use, maintenance, and management of clinical equipment and stock.

You will be a NMC/1st level registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that you should have strong communication and organisation skills and a flexible approach which will ensure you deliver positive experiences to patients at every stage of their healthcare journey.

You will have a full UK driving licence with access to a car for work purposes with Business insurance for the vehicle used. You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
