Job description

Exciting opportunity to join a thriving, developing service as a B6 team leader. This role involves overseeing the shift, managing staff, handling referrals, and prioritising visits across the Torbay and South Devon area for patients in their homes. The goal is to prevent hospital admissions and support hospital discharges. The team makes use of 3 pool cars.

We are a friendly, adaptable team dealing with various health and social needs of community patients, working at night. Regular supervision sessions, both individual and group, are held to ensure everyone’s voice is heard and learning opportunities are maximized. As a B6 team leader, you will lead these supervision sessions, supervise staff, conduct return-to-work interviews, make occupational health referrals, and support team members.

The role includes providing holistic nursing assessments, delivering high-quality care, support, advice, and information to patients and carers, aiming to maintain patients in their homes and promote independence and quality of life.

You will ensure effective coordination and provision of nursing services in the absence of the Zone Nurse Lead, participating in assessments, planning, and discharge processes for complex patients transitioning from secondary to primary care.

The responsibilities include:

  • Communicating effectively with the integrated health and social care team, voluntary agencies, secondary care, GPs, carers, and relatives.
  • Working across intermediate care and community nursing teams to provide crisis response nursing interventions, assessments, referrals, coordination, delegation, and placement to prevent hospital admissions. Supporting hospital discharges and providing clinical advice to the multidisciplinary team.
  • Supporting team members through supervision, 1-1s, occupational health and physiotherapy referrals, and conflict resolution.
  • Leading by example, being approachable, self-aware, and understanding how behaviour impacts team dynamics and patient care.
  • Managing the shift, including taking referrals, prioritizing, and allocating workload based on visit locations and team member availability.
  • Participating in meetings with universities, PPG, audits, stock control, and staff management to ensure smooth service operation.
