We are looking to recruit experienced and motivated community nurses tocomplete the Specialist Community qualification (SPQ) at a level 7 DistrictNursing PgDip apprenticeship studying with Manchester Metropolitan Universityand undertaking practice placement experience within Halton and WarringtonDistrict Nursing. The course also includes the opportunity to study for theV300 Nurse Prescribing and physical examinations skills. The District Nursingservice is commissioned to deliver safe and high-quality care to houseboundpatients within the community. As a District Nurse trainee, you will be workingtowards developing essential skills and knowledge to care for patients withlong term conditions and complex health needs within an integrated healthcare structure.There will be exciting opportunities to develop strong leadership and clinicalskills to ensure the delivery of effective evidence-based practice, clinicalassessment, care planning and caseload management.

Nursing / MidwiferyQualifications and your NMC pin.
GCSE English andMaths at Grade C or above or evidence of an equivalent English and Mathsqualification.
Whilst undertaking the SPQ over the first year (52 weeks and 1 day) thesuccessful candidates will be renumerated at a band 5 then, subject to thesuccessful completion of the course requirements, move onto a band 6 DistrictNurse Team Leader.
The District Nursing service is commissioned to deliver safe andhigh-quality care to housebound patients within the community. As a DistrictNurse trainee, you will be working towards developing essential skills andknowledge to care for patients with long term conditions and complex healthneeds within an integrated healthcare structure. There will be excitingopportunities to develop strong leadership and clinical skills to ensure thedelivery of effective evidence-based practice, clinical assessment, careplanning and caseload management.

Perform holisticassessments of patient nursing needs. Plan, assess, implement, and evaluatecare delivery according to changing health needs utilising prescribing skills.
Support the caremanagement of complex/continuing care packages.
Ensure practiceis evidence based through undertaking audit and critically appraisingliterature to achieve a high standard of care.
To establish andmaintain effective communication with stake holders, individual groups andcommunities in complex and potentially challenging situations.
Involve patientsand carers in the planning and delivery of care.
To competentlyperform a range of clinical skills related to the post.
Maintainaccurate records in accordance with NMC Standards.
To participatein the health needs assessment of the practice population to identify areas ofunmet need.
Ability to workwithout direct supervision and to solve problems.
Undertakechronic disease management reviews of the housebound patient using appropriatetemplate
Work on a rotabasis to facilitate weekend working across the district nursing teams.
