Job summary

Livewell Southwest Tavistock community nursing team seeks a Band 5/6 Community Sister/Charge Nurse for a 37.5 hour per week fixed term contract. You will lead a nursing team, assess complex cases, and collaborate across primary and secondary care to deliver holistic, home-based care.

You will manage a clinical caseload, teach and support staff, and travel between Livewell Southwest sites and community locations. Fixed term or secondment basis with potential development opportunities.

Qualifications

• Current NMC registered nurse with Band 5/6 experience.
• Mentoring experience preferred.
• Leadership skills and ability to manage a team.
• Strong clinical assessment and decision-making abilities.

Responsibilities

• Assess and treat patients with complex health and social care needs.
• Lead, supervise and develop the nursing team.
• Coordinate care with internal and external services and MDT discussions.
• Maintain accurate records and participate in clinical governance.
• Provide clinical supervision to staff and students.
