Job summary

Livewell Southwest in Tavistock invites applications for a Community Sister/Charge Nurse (Band 6) on a 12-month fixed-term contract. The role is full-time, 37.5 hours per week, working with the Tavistock Community Nurses across primary and secondary care settings.

You will lead a nursing team, assess and manage complex cases, and travel between Livewell Southwest sites and community locations. The post requires leadership, autonomous practice, and commitment to high standards of care.

Qualifications

• Essential: First Level Nurse Registration (degree or diploma).
• Essential: NMC Registration.
• Current registered nurse with relevant experience.
• Mentoring experience desirable.

Responsibilities

• Assess, triage, plan, implement and evaluate care which may be complex with senior support.
• Coordinate care delivery and apply clinical judgement during day-to-day changes.
• Develop nursing skills and competencies to enhance the team.
• Maintain confidential, accurate records in paper or IT systems.
• Provide clinical supervision to staff and students.
• Promote health, support independence and health promotion.
• Work across geographical areas delivering nursing duties within community and clinics.
