Job summary

Aneurin Bevan University Health Board in Wales is seeking a Band 5 Community Nurse to provide high-quality care in patients' homes and community settings from the Abergavenny area. You will be supported by induction and CPD training to enhance your clinical skills.

You will autonomously assess and deliver care while collaborating with GPs, social services and multidisciplinary teams, with emphasis on patient independence and safe practice. Documentation will be meticulously recorded.

Qualifications

• UK NMC Registered Nurse
• Evidence of continuing professional development
• Evidence of portfolio development
• Able to travel between sites in a timely manner

Responsibilities

• Assess, plan and deliver high-quality, evidence-based nursing care in patients' homes and community settings
• Manage a daily clinical caseload with minimal direct supervision
• Collaborate with GPs, social services and MDTs to ensure integrated care
• Maintain accurate documentation for each visit to uphold safety and continuity
