Job summary

The Methodist Church in Britain is recruiting a community staff nurse for the Community Intervention Team in South West Staffordshire, to provide hands-on nursing care in patients' homes to help keep people out of hospital.

The role covers IV antibiotics, wound care, palliative care and venepuncture, with referrals from ambulance services, GPs and hospital staff. Requires nursing degree, current NMC registration and mentorship qualification.

Qualifications

• Nursing degree and mentorship qualification.
• Registered adult nurse with current NMC registration.
• Experience in community nursing or acute care.

Responsibilities

• Assess, plan, deliver and evaluate evidence-based nursing care for patients in their homes.
• Support patients to self-care and live independently at home.
• Collaborate with multi-professional colleagues to prevent admissions and enable discharge.
• Take referrals from ambulance services, GPs and hospital staff.
• Provide IV antibiotics, wound care, injections, PEG feeds, venepuncture and palliative care.
• Deliver general nursing care across patient needs.
