Job summary

Wye Valley NHS Trust is seeking a Community Staff Nurse to join the Golden Valley District Nursing Team. You will provide home-based care, including wounds, palliative support, and long-term condition management, travelling across a large rural patch.

Newly qualified nurses are welcome if they gain a mentor qualification within 12 months. Registration as an Adult Nurse is required and you will work with GPs, palliative teams and therapists to keep patients independent and out of hospital.

Qualifications

• Registered Nurse Level 1 (Adult) registration in good standing.
• Mentor and assessing qualification within 12 months for new staff.

Responsibilities

• Perform holistic nursing assessments in patients' homes.
• Deliver wound care, including negative pressure therapy.
• Manage wound assessment and ongoing care.
• Provide palliative care and end-of-life symptom management.
• Support relatives and carers of patients.
• Manage long-term conditions in community settings.
• Handle CVAD/drain management and chemotherapy disconnections.
• Take venous blood samples and support patient independence.
