Job summary

• Undertake holistic assessment including complex presentations.

• Promote independence and reduce hospital admissions where possible.
• Contribute to risk assessments and health and safety.
• Communicate effectively with patients, carers and colleagues.
• Participate in MDT meetings and liaise with other agencies.
• Maintain accurate and timely written records in line with Trust policy and NMC guidance.
