Job summary

Cornwall Partnership NHS Foundation Trust invites applications for a Community Nurse (Band 5) based with the Camborne Community Nursing Team. The post is open to newly qualified professionals and focuses on delivering high-quality community care within the Camborne area.

You will work 37.5 hours per week across evenings, weekends and bank holidays, with a mix of full-time and part-time arrangements. A supportive team and opportunities for development are provided.

Qualifications

• Registered Nurse (Adult) with current NMC registration.
• Evidence of recent relevant professional development.
• Mentor for student placements or willing to work towards mentoring qualification.
• Hold a post-registration diploma or be working towards a degree.

Responsibilities

• Assessment, planning, implementation and evaluation of care, without direct supervision, in patients' homes.
• Work in partnership with patients and carers to offer support and informed choice.
• Support the Community Nurse Team Leader.
• Provide evidence-based care in line with policies and training.
• Support locality teams by flexible deployment as service demands.
