Job summary

Kent Community Health NHS Foundation Trust is seeking a Neighbourhood Nurse to support frontline NHS clinical teams in delivering patient care across the community. The post involves signposting to other services, staying updated with evidence-based practices and using IT systems and databases.

A full UK driving licence and independent travel are essential. The role is open to staff of Kent and Medway bodies and new graduates may apply.

Qualifications

• Level 6 degree or equivalent qualification.
• Diploma in nursing if no degree.
• NMC registration.
• Recent healthcare experience gained through education, work experience or previous employment.

Responsibilities

• Provide support to frontline NHS clinical teams in delivering patient care.
• Signpost clients and families to alternative services.
• Keep clinical practice up to date using evidence-based procedures and clinical effectiveness frameworks.
• Use IT systems and databases.
• Contribute to open review of practice, team working and sharing good practice to improve care.
