The role offers varied community placements, with an induction plan to ease into the community environment and to support ongoing professional development.

The team focuses on diabetic and wound care, catheterisation and continence, and palliative care, with emphasis on rehabilitation, self-management and independence across the care continuum.

• Diabetic management
• Wound management including leg ulcers diagnosis and treatment
• Palliative/end of life care including catheterisation and continence needs
• Promote independence and self-management in rehabilitation
• Work in partnership with local health and social care providers to deliver high-quality community care
• Support colleagues with evidence-based practice and staff development
