The Sussex Community NHS Foundation Trust is seeking a Community Staff Nurse in Crawley, based at Crawley Hospital. The role covers diabetes, wound and continence care, palliative support, and frailty caseloads with local partners. An induction plan is included for new starters.

You will work with a team to deliver evidence-based care and support patient independence across the care continuum, with opportunities for professional development and flexible working options.

Competenze

• RN qualification, degree or diploma.
• ENB 998 or equivalent, or working towards it.
• NMC registration.

Mansioni

• Diabetic management within primary care.
• Wound management including leg ulcers.
• Palliative and end-of-life care.
• Bladder and bowel care including catheters.
• Frailty support with local providers.
• Assist rehabilitation, self-management and independence.
