Job summary

Buckinghamshire Healthcare NHS Trust is seeking two Community Staff Nurses to join our community service in High Wycombe. The role focuses on supporting Healthcare Assistants to deliver high-quality care in patients' homes, including long-term conditions reviews, phlebotomy and continence assessments.

We welcome newly qualified and experienced nurses, offering training and professional development within a 7-day service pattern and flexible working options.

Qualifications

• Registered Nurse must be on NMC register or awaiting PIN.
• Evidence of ongoing professional development (CPD).
• Mentoring qualification or progression towards it is desirable.

Responsibilities

• Contribute to service and quality improvement processes.
• Assess patients' physical and psycho-social needs and provide evidence-based interventions.
• Manage own caseload and deliver care with supervision.
• Plan interventions with Case Manager and team using shared assessment process.
• Participate in referral management to ensure appropriate care by the right clinician.
• Contribute to development of clinical pathways and protocols.

We have an exciting opportunity for two Community Staff Nurses to join our service in a new and innovative role working across our seven locality teams.

This role offers a different way of working, with a particular focus on supporting Healthcare Assistants to deliver high-quality care in patients' homes. Key areas of work include long-term condition reviews, phlebotomy, continence assessments and promoting patient independence to prevent hospital admission and support timely discharge.

Working closely with the wider multidisciplinary team, you will be expected to demonstrate initiative, embrace new ways of working and contribute to the delivery of safe, effective, patient-centred care. We welcome applications from both newly qualified and experienced nurses and offer excellent opportunities for training and professional development.

The service operates across a 7-day working pattern.

Main duties of the job

• To contribute to processes for service and quality improvement
• Assess the physical and psycho–social needs of patients and deliver
  therapeutic interventions based on best available evidence in order to improve health outcomes.
• To manage own case load, delivering and evaluating service delivery to patients with a variety of medical conditions and those delegated by case managers.
• To plan interventions with the Case Manager and other team members using the common assessment process
• To take part in the referral management process to ensure patients are treated by the most appropriate member of the team/service
• To contribute to the development of protocols and procedures developed for clinical pathways

EDUCATION, QUALIFICATION & TRAINING
