Are you interested in being part of an innovative, caring, and agile Community Nursing team? Do you want to work with an award-winning team? Welwyn and Hatfield Integrated Care Team, winners of the Hertfordshire Community Trust Leading Lights awards for Demonstrating Fantastic Patient Care, are seeking band 5 Nurses to join us.
Hertfordshire Community Trust, rated 'Good' by CQC in May 2020 and working towards 'Outstanding', delivers high-quality nursing care across Welwyn and Hatfield. We focus on preventing hospital admissions, facilitating discharges, providing planned nursing interventions, and managing long-term conditions.
The successful candidate will be supported in developing skills and will join a team committed to demonstrating our trust values, caring for each other as well as our patients. We collaborate with our partner organizations across East and North ICB, Hertfordshire Foundation Trust, and Hertfordshire.
Our team operates within the Welwyn and Hatfield Locality. The HCT Integrated Community Care Teams are dedicated to delivering evidence-based nursing care in patients' homes and support patient discharge and hospital admission prevention through an integrated multidisciplinary approach.
We offer a comprehensive induction program and the opportunity to work in a friendly, dynamic, professional, and caring environment. Candidates should have the ability to creatively problem-solve and work independently, with an understanding of current professional and community issues. Community nursing experience is preferred, but training and development opportunities are available for the right candidates.
