Job summary

Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust is seeking a Band 5 registered nurse to join the Newcastle North and East Community Treatment Team. You will deliver person-centred care in home and community settings, supporting recovery and independence within a multi-disciplinary team.

You will conduct holistic assessments, plan and evaluate care, and support depot and clozapine clinics in line with guidelines, ensuring safety and continuity of care for service users and carers.

Qualifications

• Registration as a first level nurse (RMN/RNMH/RNLD) with current live registration.
• Experience delivering treatment plans with service users and carers.
• Complete preceptorship programme for newly qualified nurses.
• Proven ability to work within a multi-disciplinary team.

Responsibilities

• Undertake comprehensive mental health and risk assessments, contributing to care planning and reviews.
• Deliver therapy and psychosocial interventions per care plans and recovery-focused approaches.
• Administer and monitor depot injections in clinic and community settings.
• Support clozapine clinics, including health monitoring and blood test compliance.
• Monitor mental state, medication concordance and physical health; escalate when needed.
• Develop therapeutic relationships to support engagement and recovery.
• Work with service users, carers, families and partner agencies.
• Participate in safeguarding and risk management, including crisis response.

An exciting opportunity has arisen for a Band 5 registered nurse to work within the Newcastle North and East CTT.

The Band 5 Mental Health Community Nurse is responsible for delivering high-quality, person-centred care within CTT, supporting individuals living with a range of mental health needs in a community setting. The role involves working collaboratively within a multi-disciplinary team to promote recovery, independence, and wellbeing, while reducing hospital admissions and supporting early discharge.

The post holder will manage a defined caseload, providing holistic assessment, planning, implementation, and evaluation of care in line with evidence-based practice and organisational policies. Care is delivered in service users' homes, clinics, and other community settings, ensuring accessibility and continuity of care.

The role also includes participation in depot and clozapine clinics, ensuring safe administration, monitoring, and follow-up in line with local and national guidelines.

Main duties of the job

Essential criteria

• Experience of working with service users and carers in successful delivery of treatment plans
