Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust (CNTW) is hiring a Band 6 Community Practitioner on a fixed-term 18-month contract. The role involves leading in a multidisciplinary team, providing recovery-focused care, and coordinating complex case management for service users with criminal justice links.

The post requires current RN registration and post-registration training in clinical/psychological interventions, with opportunities for leadership within an established MDT.

Qualifications

• Professional qualification and current registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent.
• Post-registration clinical/psychological interventions training.
• Experience in a community team setting.
• Training in CPA, Safeguarding, MAPPA and Child Protection.
• Up-to-date knowledge of relevant legislation and non-medical prescribing.

Responsibilities

• Assess and manage caseload of service users with substantial complex & criminal justice needs.
• Provide specialist clinical advice to service users, carers, staff, agencies and the public.
• Develop Recovery and Wellbeing focused Care Plans with regular reviews.
• Deliver evidenced based Clinical and Psychologically based Interventions.
• Provide clinical supervision to staff and coordinate treatment planning and discharge planning.

North Tyneside Recovery Partnership have an exciting opportunity for a Band 6 Nurse to join the team, working into the criminal justice pathway.

The post is initially a fixed term 18 month contract with potential for further extension

The role entails working with those affected by substance & alcohol misuse who also have criminal justice connection to provide safe and effective interventions, harm reduction advice and use of psycho social interventions to support service users to consider and achieve recovery. The role involves individualised clinical assessment, care planning, formulation, and collaborative working across the multi-agency team.

The team consists of a well-established mix of professionals that work collaboratively to deliver individualised care.

The applicant will be expected to demonstrate leadership within the team, providing supervision and supporting students.

Main duties of the job

To be responsible for the Assessment and Management of casework of service users who have substantial complex & criminal justice needs.

Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.

In collaboration with the Service User & Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.

To deliver evidenced based Clinical and Psychologically based Interventions.

The post holder will work as a member of the MDT ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.

Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning. Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager.

To provide compassionate care that is based on empathy, kindness, respect and dignity.

To incorporate a client centred philosophy into nursing practice and client centred collaborative care.

Education and Qualification

• Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
