We are seeking dynamic and innovative Band 5 Community Staff Nurses to join the East Locality Community Nursing Team within the Integrated Nursing and Conditions services at CHCP CIC. This position is based at Longhill Health Centre, offering 30 hours per week.

Role Overview

The successful candidates will be part of an integrated community nursing team providing holistic, high-quality care to a dedicated client group in their homes and residential settings. You will work collaboratively with health and social care providers, developing new ways of working and working independently.
