Cheshire and Wirral Partnership NHS Foundation Trust is seeking a Band 5 Community Staff Nurse to join the Chester East Care Community Team. The role includes delivering high-quality community nursing services and home visits across the West Cheshire area.

The post is 22.5 hours per week with weekend rotation, permanent contract, and a pro rata salary of £32,073–£39,043 per annum. Training and induction support are provided to help you settle in.

Qualifications

  • NMC registration as a General Nurse (Part 1).
  • Evidence of recent clinical training or update in practice.
  • Working towards or holding a Degree in Professional Practice.

We have an exciting opportunity for a Band 5 enthusiastic, motivated, and keen nurse to join our established multidisciplinary Care Community Team at Chester East to support and deliver a range of quality nursing services to our patients.

Main duties of the job

Our eight care community teams are based in various locations within Chester, Ellesmere Port, Neston, Frodsham and the Rural locality covering the geographic area of West Cheshire. You will need access to transport or have the ability to travel to a variety of venues as part of the working day. Mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidates will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
