Tees, Esk And Wear Valleys NHS Foundation Trust is seeking a Mental Health Wellbeing Practitioner to deliver psychologically informed interventions in the County Durham community. You will work under supervision within a multidisciplinary team to support adults in planning and maintaining recovery and wellbeing.

The role requires graduate/postgraduate level certificates in Mental Health and Wellbeing, group facilitation experience, and strong collaboration with partners across services.

Formación

• Experience delivering psychologically informed wellbeing interventions.
• Ability to work under supervision and with multidisciplinary teams.
• Experience of working in community mental health settings and care planning.

Responsabilidades

• Deliver psychologically informed wellbeing interventions.
• Support adults to engage in care planning and risk assessment with the team.
• Collaborate with partner organisations to provide holistic community support.
• Contribute to multi-disciplinary care plans and follow supervision guidance.
