Job summary

EPUT is recruiting a Band 6 Community Social Worker for the Basildon Mental Health Unit. The post involves full-time hours (37.5 per week) with a shift pattern including weekends and bank holidays.

You will assess, plan and implement care for adults and older adults with mental health needs within the Crisis Resolution Home Treatment Team. The role requires a professional qualification (OT, Social Work, or RMN), ASYE completion, and may include dementia-related qualifications.

Qualifications

• Professional qualification in OT, Social Work or RMN.
• Completion of ASYE (Assessed and Supported Year in Employment).
• Additional professional qualification/experience related to dementia.

Responsibilities

• Establish and maintain effective health and social care for adults and older adults with mental health needs.
• Assess client needs, plan, implement and evaluate interventions.
• Document care accurately and concisely.
• Participate in initial assessments for appropriateness of Crisis Resolution Home Treatment Service.
• Attend and contribute to team and clinical meetings; discuss new care models and trials.
• Be mindful of values and attitudes to ensure non-judgmental, objective care.
• Develop therapeutic relationships with clients and families.
• Ensure confidential communication between agencies and maintain secure records.
