Job summary

Greater Manchester Mental Health NHS Foundation Trust is recruiting 10 Mental Health Practitioners to join our community teams across Manchester. You will provide timely interventions for service users with enduring mental health needs, working within the MDT to deliver patient-centered care.

You will manage a defined caseload, follow Care Programme Approach procedures, and contribute to service improvement while collaborating with service users and carers.

Responsibilities

• Be a full member of the MDT.
• Manage a defined caseload of clients in the community.
• Record interventions in line with clinical pathways.
• Ensure compliance with Care Programme Approach.
