Job summary

Surrey and Borders Partnership NHS Foundation Trust is seeking an MHP within a neighbourhood team to deliver person-centred mental health care in the community. You will collaborate with statutory, voluntary, and community organisations to provide coordinated services close to home.

You will undertake comprehensive mental health and risk assessments, deliver evidence-based interventions, and lead care coordination for complex cases, ensuring safeguarding and governance requirements are met while

Responsibilities

• Undertake comprehensive mental health assessments and risk assessments.
• Deliver evidence-based nursing interventions and treatment plans.
• Act as a Care Coordinator/Lead Professional for individuals with complex mental health needs.
• Work collaboratively with patients, carers, families, and partner agencies.
• Promote recovery-focused and person-centred care.
• Contribute to safeguarding, risk management and clinical governance requirements.
• Support service development and quality improvement initiatives.
• Demonstrate good communications skills and able to work collaboratively within a multi-agency team. Subject to reasonable adjustments in respect of any disability, candidates must possess a valid driving licence and be able to drive and have the unrestricted use of a car or motorcycle for work at all times including community based work.
