Job summary

Sussex Partnership NHS Foundation Trust in Chichester is recruiting a Community Mental Health Practitioner to support people with complex mental health needs within a multidisciplinary Neighbourhood team. You will carry a defined caseload and deliver comprehensive assessments, care planning and recovery-focused interventions.

The role offers flexible working, training, supervision and opportunities to mentor juniors, with a focus on person-centered, recovery-oriented care and partnership working

Qualifications

• Registered Mental Health qualification or equivalent.
• Post-qualification CPD and specialist training relevant to community mental health.
• Experience managing a clinical caseload in a multidisciplinary setting.
• Experience conducting comprehensive risk assessments and delivering interventions.

Responsibilities

• Undertake comprehensive mental health assessments and manage risk.
• Develop, implement and review care and recovery plans with service users.
• Deliver evidence-based interventions tailored to individual needs.
• Coordinate care and support transitions between services.
• Work with families and carers to promote recovery and independence.
• Provide supervision and mentorship to junior colleagues and students.
