Job summary

Sussex Partnership NHS Foundation Trust in Chichester is recruiting a Community Mental Health Practitioner to support people with complex mental health needs within a multidisciplinary Neighbourhood team. You will carry a defined caseload and deliver comprehensive assessments, care planning and recovery-focused interventions.

The role offers flexible working, training, supervision and opportunities to mentor juniors, with a focus on person-centered, recovery-oriented care and partnership working

Qualifications

• Registered Mental Health qualification or equivalent.
• Post-qualification CPD and specialist training relevant to community mental health.

Who can apply

We’re looking for a professionally registered practitioner with:

• A Registered Mental Health qualification or equivalent
• Relevant post-basic courses, study days and evidence of continuing professional development
• Post-qualifying experience working with people experiencing a range of mental health difficulties
• Experience successfully managing and prioritising a clinical caseload
• Experience undertaking comprehensive clinical and risk assessments
• Experience delivering a range of clinical interventions
• Experience working collaboratively with statutory and non-statutory services and agencies
• Confidence working consultatively with other professionals

The role

As a Community Mental Health Practitioner, you’ll hold responsibility for a defined caseload of people with complex mental health needs, providing safe, effective and evidence-based support within the community.

You’ll:

• Undertake comprehensive mental health assessments, including assessment and management of risk
• Develop, implement and review collaborative care and recovery plans
• Deliver evidence-based interventions tailored to individual needs and goals
• Coordinate care and support people through transitions between services
• Work closely with service users, families and carers to promote recovery and independence
• Provide psychoeducation and specialist clinical advice
• Maintain effective risk management and safety plans
• Work collaboratively with colleagues across the multidisciplinary team and partner organisations
• Maintain accurate, timely and comprehensive clinical records
• Contribute to service development, quality improvement and the delivery of high standards of care
• Provide supervision, mentorship and support to junior colleagues, students and trainees

Working in the Chichester Neighbourhood Mental Health Team

You’ll work alongside colleagues from different professional backgrounds to provide coordinated community-based care for people experiencing complex mental health difficulties.

The role combines specialist assessment and intervention with care coordination, risk management and partnership working. You’ll work closely with service users and their support networks to identify strengths, agree meaningful goals and help people achieve greater independence and positive clinical outcomes.
