Job summary

Dorset HealthCare in Poole invites applications for a Band 6 Community Mental Health Practitioner to join the Poole Community Mental Health Team on a full-time basis (37.5 hours per week, Monday to Friday, 9:00am-5:00pm).

The postholder will work within an integrated multidisciplinary team, undertake comprehensive assessment, care planning, risk management and multi-agency liaison to provide safe, person-centred care and support recovery journeys.

Qualifications

• Recent experience of working in a mental health environment.
• Ability to apply engagement and recovery philosophies in practice.
• Excellent communication and organisational skills with the ability to prioritise work.

Responsibilities

• Provide care coordination across the recovery journey.
• Undertake comprehensive assessment and care planning.
• Manage risk and liaise with community, primary, and secondary/tertiary services.
