Due to maternity leave, we have an exciting opportunity for a part-time, fixed-term (9 months) Band 6 role within our Older Adults CMHT and Memory Service.
The team supports individuals with mental health problems and undertakes memory assessments.
We cover the Vale Royal area, including Northwich and Winsford. We are looking for a qualified mental health nurse.
We are ideally looking for a nurse with both experience of functional mental health and memory assessments. However, we would consider a nurse with experience of one of these areas.

This is a great time to be joining the team, as you will be part of developing our response to Community Mental Health Transformation.
The post holder will work autonomously within a multi-disciplinary framework, undertaking assessments and coordinating the care of patients with both functional and organic mental illness, as well as undertaking memory assessments.
The post holder will maintain a caseload which will involve ongoing assessment: Formulation of care planning, risk assessments and memory assessments. Completing reviews and offering interventions to help improve wellbeing/outcomes for the individual and their families, signposting to other agencies as necessary. Excellent communication, report writing, and assessment skills are essential for this post. Applicants should have sound working knowledge of mental health disorders and the Mental Health Act.
