Job summary

Oxford Health NHS Foundation Trust is seeking a passionate Psychiatric Nurse to join the Oak Team in community mental health services. You will be the allocated key worker for a caseload of patients with severe mental illness, conducting assessments, planning care, and coordinating with a multidisciplinary team.

You will perform bio-psycho-social assessments and deliver brief interventions, with home visits and travel across Trust sites.

Qualifications

• Postgraduate education in community care or equivalent
• Professional registration (NMC)
• CPD portfolio

Responsibilities

• Serve as the allocated key worker for a caseload of patients with severe and enduring mental illness
• Conduct bio-psycho-social assessments and develop collaborative care plans
• Deliver brief evidence-based interventions up to eight weeks and coordinate with families/caregivers
• Complete duty triage of new referrals and assess need for AMHT involvement
• Identify and escalate safeguarding concerns and manage risks
• Collaborate with partner agencies to support patient independence and community inclusion

Oak Team, part of the City and North East Oxfordshire AMHT, offers a fantastic opportunity to join a well-established service delivering care to a diverse and vibrant population across Oxford. Our service includes three locality-based teams aligned with GP practices. You will join a dynamic multi-disciplinary team, working alongside a range of clinical professionals and closely with third sector partners to provide high-quality, holistic, person-centred care. This is a full-time role (37.5 hours, Monday-Friday, 09:00-17:00). Due to the nature of the role, the ability to travel independently across Trust sites and to conduct home visits is essential and not all areas are accessible via public transport. Join us and play a key role in shaping the future of community mental health care., The role will involve being the allocated key worker (formerly known as Care Coordinator) for a caseload of patients with severe and enduring mental illness. In these instances, you will be playing an integral role as part of a multi-disciplinary team in that person's recovery, which would include the assessment, collaborative care planning and review of the person's needs. You will also be expected to undertake bio-psycho-social assessments for patients referred to the team, and this may include offering brief evidence based interventions for up to eight weeks, ensuring to work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community. This may include working closely with partner agencies.You will also be expected to complete duty, which includes triaging new referrals (we receive referrals from the Keystone Hubs, GPs, EDPS etc) and identifying if the patient requires an assessment within the AMHT or signposting to other services. You will be expected to be able to complete risks assessments and appropriately escalation any concerns about risk or safeguarding., We hope that the advert has given you a clear understanding of the skills we are seeking and the opportunity at hand.

• Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management)
  Professional Qualification (Degree or equivalent)
• Evidence of Continuing Professional Development (CPD) portfolio
• Professional Registration, To have experience of working within a multidisciplinary team
• To have highly extensive experience which should include community and working with mental health
• To have a good knowledge and interest in community care and have specific clinical expertise in mental health
• Understand and have experience of Risk Management
• Experience of supervising other staff
• Understand and have experience of Risk Management, Good knowledge of community services
• Awareness of National and local issues concerning mental health
• Experience of supervising student placements.
