A leading NHS organization in Lichfield seeks a NMC Registered Band 5 Community Mental Health Nurse. This role involves delivering specialist assessments and interventions for older adults with mental health issues. Join our Older Adult Community Mental Health Team as a NMC Registered Band 5 Community Mental Health Nurse based in South Staffordshire. As a band 5 Community Mental Health Nurse you will be part of a dedicated service that will provide specialist input to service users and their carers who may be affected by a mental health diagnosis. You will be responsible for the delivery of specialist assessments and evidence-based interventions associated to all areas of older adults' mental health and dementia and contribute to delivering high-quality clinical services. You will hold a designated caseload, ensuring a comprehensive and tailored approach to care, engage in therapeutic interventions, following meticulous care plans, and making a positive impact on the lives of those navigating mental health-related difficulties. Flexibility is your forte as you navigate unpredictable work patterns and client bases, offering compassionate care in highly distressing situations. Assume the role of educator and advisor, making significant contributions to the supervision of junior staff. This role is for 18.75 hours per week and as part of this role you will be required to travel across a large geographical area. This role does not meet eligibility for sponsorship.
Your journey begins in the Memory and Dementia Team, where you'll contribute to delivering high-quality clinical services to service users and their families. Collaborate seamlessly with fellow providers, aligning with national strategies and NICE Guidelines. Take charge of a complex caseload, offering clinical and caseload supervision to support your team. As a keyworker (formerly known as a care coordinator), weave together a tapestry of communication and coordination for your designated caseload, ensuring a comprehensive and tailored approach to care. Engage in therapeutic interventions, following meticulous care plans, and making a positive impact on the lives of those navigating mental health-related difficulties.
Simultaneously, your role extends to the Older Adults Team, where you'll shape the well-being of the elderly population in South Staffordshire. Work in tandem with Staffordshire Police and Mental Health Services, embodying the Trust's commitment to people-centric care. Leverage your extensive experience in older adult and dementia care, conducting assessments, formulating person-centred care plans, and managing risk assessments. Flexibility is your forte as you navigate unpredictable work patterns and client bases, offering compassionate care in highly distressing situations. Assume the role of educator and advisor, making significant contributions to the supervision of junior staff. Contribute to the optimization of service users' well-being, ensuring timely discharges and seamless transfers.
