The Community Mental Health Team is seeking a skilled, motivated, organised, and enthusiastic Mental Health Nurse to work within an established integrated, multi-disciplinary team, focusing on individuals with severe and enduring mental health problems. The successful post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
