As part of the innovative and ongoing development we are looking for an experienced practitioner to join our Acute Frailty Team.
As the population ages there are increasing numbers of older people living with frailty who are presenting to ED departments. It is recognised that targeting this high risk group as early as possible during admission provides an opportunity to improve quality of care and optimise patient and service outcomes for a vulnerable population.
The role involves working in the Emergency Department and the Frailty Assessment Unit (FAU) as well as the development of rapid access and ambulatory clinics.
Excellent and advanced clinical knowledge of caring for those identified as frail is essential with the determination and drive to make a difference.
To take the lead in Ambulatory / Front Door Frailty and same day emergency care
Examination and diagnostics – referring when appropriate to members of the MDT.
To ensure appropriate diagnostic ordering and review.
To perform clinical skills - cannulation / venepuncture, ABG’s, blood cultures, catheterisation.
To safely prescribe medications and perform medication review. Completing VTE and dementia screens. Ordering TTO’s, writing discharge letters.
To support the AIFU and Consultant In - Reach service by assisting in ward rounds.
To provide a consistent and personalised service by ensuring that the patients receive the best care, in the best place, from the best person or team.
To provide social, emotional, and psychological support to patients and their carers/relatives from referral and throughout their treatment.
To autonomously manage a patient caseload efficiently and timely, prioritising tasks.
To make appropriate referrals to consultants/other specialists/ members of the multidisciplinary team in primary, secondary, or tertiary care settings.
To provide teaching as an in-house measure to all medical and nursing staff, not only within the Directorate but also from other services.
To partake in service development through audit and data collection.
To maintain personal accountability for professional and ethical actions and ensure compliance with NMC code of professional conduct.
