Oxleas NHS Foundation Trust is seeking a Mental Health Nurse to join the Early Intervention in Psychosis Team (Erith area). The role involves supporting people recovering from first-episode psychosis within community settings and home environments.

You will work as part of a multidisciplinary team under supervision, delivering evidence-based care and health education with flexible support for service users.

Qualifications

• NMC Registration Essential.
• Experience of working with psychosis.
• Experience of working in community mental health (placement).
• Experience of working with Dual Diagnosis.
• 2 years Community Experience.
• Experience of supervising junior staff.
• Experience of liaising with other agencies.
• Experience of EIP (Early Intervention in Psychosis).
• Knowledge of Clozapine & Depot medication management.
• Experience of facilitating /planning / supporting groups in a healthcare setting.

Responsabilités

• Deliver evidence-based mental health nursing care to clients in home, community locations, clinical environments and care settings.
• Assess, plan, implement and evaluate patient care within a multi-disciplinary community team under supervision.
• Maintain links between primary and secondary care, social services and voluntary agencies.
• Provide health education for patients, carers and families; establish professional relationships.
• Ensure up-to-date mandatory training per Oxleas Foundation NHS Trust guidelines.
• Deliver care and formulate personalised care plans; implement and evaluate care.
