Are you a Mental Health Nurse who has a passion to work within OPMH and want to join a friendly team to do this?

The New Forest West Older Persons Community Mental Health Team is based at South Efford House. As a community mental health practitioner your journey will take you around the beautiful New Forest and along the coast delivering high quality care and interventions to our patient group.

There is an opportunity to work within our CMHT pathway working with both organic and functional illness, In reach within our local care homes and nursing homes, and also work within our memory assessment service. If you have a keen interest in any of these areas, then now is the time to join and influence the development and delivery of these pathways to help shape the future excellent standard of care for our patients and their carers.

The supportive leadership team promote the development of our services to provide the best outcomes for our patients and we know that to achieve this we need to invest in our staff through creating a good working environment and providing training opportunities. We are also open to flexible ways of working with hybrid working (a mix of face to face and online/telephone assessments) available to help balance your work and homelife.

Main duties of the job

To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.

To participate within the multi-disciplinary professional team in the relevant care group to deliver safe patient care.

To be an active participant in the multi-disciplinary team, formulating care plans and utilising appropriate assessments.

To work in partnership with families and carers to ensure the patient's individual health needs are met.

Understand evidence based therapeutic interventions relevant to the care group to include education, clinical care, wellness recovery action plans and discharge planning.

To participate in regular clinical, professional, and managerial supervision.

To supervise junior staff.
